The Social Atlas forms part of the CCNAP's three-year work programme on mapping need for community care services in the area. The first volume is made up of existing population and socio-economic data which is likely to be of relevance to commissioners of community care services. A selection of maps and tables from this Atlas are currently available online on this site. Work is also progressing on developing a companion volume II, which contains service activity data on people known to be in receipt of community care services as well as information on the distribution of relevant services. - Hull and the East Riding of Yorkshire Mental Health Project - Hull and the East Riding of Yorkshire The Mental Health Joint Strategy Group has commissioned CCNAP to assist them with the development of a needs based planning mechanism as required by the Department of Health. Growing old in the East Riding of Yorkshire - Matching Needs and Services This short project was commissioned by the joint planning group for Older People in the East Riding of Yorkshire. The intention is to provide health and social care agencies with; baseline information on the current needs and whereabouts of the older (75+) frail population; details of service provision and take-up amongst the older population; as well as 5-10 year projections of future support needs. Pathways Research and Supported Housing Needs CCNAP has been commissioned to undertake a series of Pathways research projects across
the region. These projects represent an attempt by the local health and social care
agencies to improve their forward planning of supported housing for vulnerable adults in
the area. Each of the Pathways research projects outlined below involve gathering a range
of primary and secondary research data from service providers, to allow estimates of
supported housing needs to be projected. The method used for the research is based on one
developed by Lynn Watson of Pathways Associates. Whilst this method does not draw on the
experiences and knowledge of service users, some supplementary fieldwork on user views is
being considered. Work in progress includes research on projecting the supported housing
needs of adults with learning disabilities in the region. Learning Disabled Adults - Supported Housing Needs in North East Lincolnshire CCNAP have recently completed a collaborative project with South Humber Health
Authority on the health and supported housing needs of learning disabled adults in North
East Lincolnshire. The report should be available on this website by October 2001. Local Evaluation of Nunsthorpe & Bradley Park Sure Start (North East Lincolnshire) The Community Care Needs Assessment Project, (CCNAP), was commissioned by Nunsthorpe and Bradley Park Sure Start to undertake a local evaluation of their Sure Start programme. Government guidance suggests that local evaluations should comprise the following: - A robust and effective monitoring system, which allows progress towards national and local targets to be measured, in way which are compatible with the Data Protection Act, 1998 and Human Rights Act, 1998. - A review of how local services are being delivered, so that successes can be built on and best practice shared, both locally and nationally. - An evaluation of local outcomes, with a particular emphasis on the short-term impact of Sure Start services on the children, families and communities the programme was designed to serve. The starting point for the local evaluation has been the establishment of a monitoring system which provides both, monthly output data on the number of children and families helped, as well as details on the type and frequency of services received. This has progressed well since its establishment in April 2000. Another key element of the local monitoring system is the routine gathering of performance data on the programmes progress towards the national Sure Start targets. CCNAP have already put in place data collection systems for most of these targets. As part of the local evaluation, CCNAP has undertaken a confidential survey of local
professionals and community workers to elicit their views on how well the local Sure Start
programme is operating to date. Impact on local children, families and the community A local community survey was completed in September 20001 by local parents and a report is available from the Sure Start office. In addition CCNAP will assist local professionals with the development of short-term outcome measures of their interventions, including information on user satisfaction.
